Transitional Youth Program

The Transitional Youth Program (TYP) supports and empowers young people aged 16-29 experiencing barriers to accessing services in the mental health sector. For up to three months, clients will receive continued therapeutic support and system navigation to ensure sustainable connections with appropriate mental health and substance use agencies in the community.

We know young people struggle to transition between mental health services as they shift from childhood into adulthood. Up to 60% of young people stop accessing care during this period. Young people do best when they receive the right supports during this time. 

Our clinician navigators and peer support workers provide flexible, timely, in-person support, whether our clients prefer to meet in the community or at our offices.

  • Address barriers to service navigation
  • Help transition youth to the right service(s)
  • Maintain motivation and engagement
  • Goal setting and skill building
  • Time-limited counselling and psychoeducation
  • Support families in helping loved ones

Referrals

We only accept internal referrals from MGH clinicians. Referrals can be made for young people between 16-29 years of age who are:

  • Experiencing barriers in accessing or maintaining services
  • Requiring continued navigation support following Michael Garron Hospital (MGH) services
  • Residing within the MGH catchment area (Bayview Ave. to Midland Ave. and Eglinton Ave. E to Lake Ontario)
  • Able to consent to services

The TYP is not a crisis service or a long-term counselling service.
