Our Outpatient Palliative Care Clinic helps patients in the community at any stage of a serious illness, supporting them to stay at home as long as possible and improving their quality of life. Connecting with us earlier in your illness helps you evaluate your goals and values, leading to informed choices as you approach the end of life.
We can meet with you to help you manage symptoms of illnesses such as cancer, chronic lung, heart or kidney diseases. This may include pain, difficulty in breathing, nausea, vomiting, fatigue, constipation, loss of appetite, anxiety and depression.
Our team supports patients who are able to attend in person clinic appointments. If you are seeking home palliative care including home visits, please speak to your primary physician and contact Ontario Health atHome. Depending on your location you may be able to access home visiting palliative services through The Temmy Latner Centre for Palliative Care or Hospice Palliative Care Team - Scarborough Centre for Healthy Communities.
