Our Health Records Team oversees the collection, storage, retention and access of patient health records. All information collected and documented in health records is according to the Public Hospitals Act of Ontario and according to professional standards that bind the various health professionals such as doctors, nurses and therapists. This Act governs the contents, retention and access to information contained in the health record.

Our Release of Information Office is responsible for the release of patient health information, including providing copies of health records to authorized persons under the Personal Health Information Protection Act (PHIPA).
Disclosure of Your Personal Health Information
The Personal Health Information Protection Act (PHIPA) allows healthcare providers to access and share personal health information for ongoing patient care. This includes the disclosure of health information to your primary care providers (e.g. family doctor or nurse practitioner) to ensure continuity and coordination of care.
Michael Garron Hospital (MGH) may assume implied consent to collect, use or disclose personal health information for the purpose of providing ongoing care. Please tell us if you do not want your primary care provider to be notified or provided information about your hospital visit.
If you wish to disclose your health records to lawyers, insurance companies and/or any other third party, please complete the Consent for Disclosure of Personal Health Information Form and mail it to the mailing address on this webpage. There is a fee applicable for this service.
Accessing Your Health Records
Under Ontario's Personal Health Information Protection Act (PHIPA), you have a right of access to your own personal health information
To access your health records online, you can sign up for MyChart during your next visit to MGH. This service is free.
To receive the copy of your health record pertaining to any visit to MGH, please complete the Request for Access to Personal Health Record Form and mail it to the mailing address on this webpage. There is a fee applicable for this service. Please refer to Frequent Asked Questions (FAQ) webpage for more information.
Correction of Personal Health Record
If you believe that your health record contains inaccurate information or is incomplete, it is your right under the Personal Health Information Protection Act (PHIPA), to request corrections to your health record.
