Health Records

The Health Records team oversees the collection, storage, retention and access of patient health records. All information collected and documented in health records is according to the Public Hospitals Act of Ontario and according to professional standards that bind the various health professionals such as doctors, nurses and therapists. This Act governs the contents, retention and access to information contained in the health record.

We are committed to protecting patient confidentiality while providing access to personal health information to authorized persons. For more information please refer to privacy.

The Release of Information section is responsible for the release of patient health information, including providing copies of health records to authorized persons under the Personal Health Information Protection Act (PHIPA).

Disclosure of Your Personal Health Information

The Personal Health Information Protection Act (PHIPA) allows healthcare providers to access and share personal health information for ongoing patient care. This includes disclosure of health information to your care primary care providers to ensure continuity and coordination of care. MGH may assume implied consent to collect, use or disclose personal health information for the purpose of providing ongoing care. Please tell us if you do not want your family doctor to be notified or provided information about your hospital visit.

If you wish to disclose your health records to lawyers, insurance companies and/or any other third party, please complete the form Consent for disclosure of Personal Health Information and mail it to Release of Information office. There is a fee applicable for this service.

Accessing Your Health Records

Under Ontario's Personal Health Information Protection Act, you have a right of access to your own personal health information

To access your health records online you can sign up for MY CHART during your next visit to Michael Garron Hospital. This service is free.

To receive the copy of your health record pertaining to any visit to Michael Garron Hospital please complete the form Request for Access to Personal Health Record. There is a fee applicable for this service. Please refer to FAQ for more details.

Correction of Personal Health Record

If you believe that your health record contains inaccurate information or is incomplete, it is your right, under Ontario's Personal Health Information Protection Act (PHIPA) to request corrections to your health record. Please complete the form Requesting Correction to Personal Health Information. For more details contact our Health Records department during business hours.

Vital Certificates

Birth Certificate: Health records Department does not issue birth certificate. Please contact The Office of the Registrar General at 1-800-461-2156 or www.ontario.ca

Death Certificate: Health records Department does not issue death certificate. Please contact The Office of the Registrar General at 1-800-461-2156 or www.ontario.ca
