Our Palliative Integrated Long-Term Care Program offers support through a palliative care Clinical Nurse Specialist (CNS) and Palliative Care Physician consultation to residents, families and long-term care teams, including personal support workers, nurses, physicians and social workers. The goal of the program is to provide palliative care support to care teams in long-term care so residents can receive care where they live and are not unnecessarily transferred to hospital.
Our team can help long term care residents who:
- Could benefit from pain and symptom management
- Have multiple chronic progressive diseases and medications
- Have had frequent hospitalizations for unmanaged symptoms such as pain, shortness of breath, agitation, nausea, vomiting
- Could benefit from an end-of-life and/or goals-of-care discussion
Long term care residents who are admitted to MGH can connect to our integrated team while still in the hospital to help them transition back to their home with a sound palliative care plan.
