REFERRAL TO PRIMARY CARE FOR ONGOING SUPPORT
After your immediate mental health needs are addressed and stabilized, the Connections Clinic will prepare and send a detailed summary of your care to your primary care provider (PCP). This summary will include information on psychiatric assessments, treatment plans, prescribed medications, and any follow-up recommendations.
This process facilitates a seamless transition to your PCP for continued medical and mental health care, ensuring that your long-term treatment is consistent, coordinated, and tailored to your ongoing needs.
Referral process
Referrals are exclusively accepted through the MGH ED; no other referrals are accepted for this service.
DROP-In TIMES
New patients are seen on a first-come, first-served basis, and space is limited. We recommend arriving toward the beginning of the drop-in hours and registering at H5.
- Mondays: 12 to 3 p.m.
- Wednesdays: 10 a.m. to 1 p.m.
- Thursdays: 12 to 3 p.m.
Preparing for your visit
Upon arrival, please check in at the front desk on H5 and you will be asked to complete a screening form, which is required before you can be seen by a clinician. To ensure a smooth visit, please bring your health card and a current list of any medications. This will assist us in providing you with the most appropriate care.
Additional supports
The Connections Clinic offers a weekly group, Building Connections. Please ask a provider in the clinic for further information. This open group provides a supportive environment for patients to build motivation, address recovery challenges, and develop skills with guidance from staff. The sessions are co-facilitated by two clinicians.
