Coxwell Entrance Closure

As of March 21, 2025, MGH’s main entrance on Coxwell Avenue is closed as the next phase of our redevelopment project begins. Patients and visitors can use the new temporary main entrance on Sammon Avenue between Coxwell Avenue and Knight Street. View our campus map.

Medical Assessment Clinic

The Medical Assessment Clinic, formerly known as the Medical Triage Clinic, provides semi-urgent assessment and care from internal medicine and infectious diseases sub-specialties. 

Patients may be referred from the Stavro Emergency Department by physicians within the hospital or from community family physicians via the SCOPE program. Patients may also receive follow-up care from this clinic after being discharged from the hospital. 

The clinic provides timely access to medicine assessment and treatment. Our clinic works with other areas of the hospital to arrange urgent tests and other specialist assessments if needed. Within our clinic, we provide care for patients with liver conditions, infectious diseases, human immunodeficiency virus (HIV) and Hepatitis B and C. 

Referrals from Primary Care

Patients require a referral from a physician or nurse practitioner (NP) to be seen by a specialist in the Medical Assessment Clinic. Referrals can be found linked on the right-hand side of this page.

If you are a referring physician or NP, please fax your form to 416-469-6253. If you wish to refer a patient to a specific physician, please indicate the physician’s name on the referral. 

PLEASE NOTE: This is not a rapid-assessment emergency clinic. Please direct your patient to the nearest Emergency Department if the reason for your referral is an emergency or an urgent medical concern.

how to prepare for your appointment

Please arrive 30 minutes before your appointment to register at Patient Registration (located on M1). Please allow 2 to 3 hours for your appointment and ensure you have arranged transportation accordingly. Please bring your health card and current medications with you.  

IMPORTANT: It is essential that you keep your appointment date and time. If you need to reschedule or cancel your appointment, please call 416-469-6580 ext. 6252 at least 48 hours in advance so we can offer your slot to another patient. Failure to notify us of a missed appointment at least 48 hours in advance may require a new referral to be seen by a specialist.
