In collaboration with Toronto Public Health, Toronto District School Board and the Ministry of Education, Michael Garron Hospital (MGH) is offering COVID-19 testing for all students at Thorncliffe Park Public School in an effort to keep the school and community safe. Testing is planned to occur on November 26, 27 and 30, 2020.
Testing will be conducted using an easy and simple mouth/nose swab, which involves rubbing the inside of the mouth and front of the nose for less than 5 seconds. A trained nurse from MGH will help your child collect the swab. Older students can self-administer the swab if they feel comfortable doing so.
Parents/guardians may register their child for testing by completing the form below. Information documented on this form is confidential and fully compliant with Ontario privacy laws. By completing this form, parents/guardians consent to their child receiving a COVID-19 test.
